

Presbyterian's MyChart gives you secure online access to your medical record. MyChart makes you an active member of your medical team and helps you keep track of all your medical information.

- For your security, you must obtain your MyChart activation code from your doctor.

If you have a PresOnline account - login in and go to step 3
- Click "register now" in the PresOnline box in right column.
- Enter your information.
- Go to step 3 when you've created your PresOnline Account

- Click the MyChart link in the Patient Services section.
- Enter your MyChart activation code exactly as it appears on the document from your primary care clinic. NOTE: use all capital letters and include the dashes.
- Click Submit.
- Read the Terms and Conditions and click Accept. You will be re-directed to the MyChart home page.
The Activation Code is a one-time-use-only code and expires after 6 months if not used. You will not use this code after you've completed the sign-up process. If you do not sign up before the expiration date, you must request a new code from your doctor.
- MyChart is NOT to be used for urgent needs. For medical emergencies, dial 911.You will use your PresOnline ID and password to login after you've completed the three steps above, not your Activation Code.
- If you need help logging in to PresOnline, call Customer Service at (505) 923-5590 or Toll Free (866) 861-7444 between 8 am - 5 pm Monday thru Friday.
- If you forget your PresOnline password at any time, use your Challenge Question to have your password sent to your email account. Once your MyChart account is activated, it is linked to your PresOnline ID, thus only your password should be reset, not your account ID.
- We introduced MyChart in 2010 at select clinics in the greater Albuquerque area. We expect it to be completely rolled out by April of 2012.

Most Presbyterian Medical Group patients can start using My Chart. My Chart is not yet available for patients of the following clinics:
- Behavioral Medicine
- Ear, Nose and Throat (ENT)
- Gastroenterology (GI)
- Pediatric Specialties
- Surgery
- Wound Clinic
- Plains Regional Medical Center outpatient clinics
- Dr. Dan C. Trigg Memorial Hospital outpatient clinics
Patients at these clinics will be able to access My Chart by the end of 2012.
